2024-2025 Family Faith Formation Registration (K-12)

Online registration has now been switched to a Google form that has been created! Unfortunately we have not been able to find a fix as of yet for My Own Church, but this will still be simple way to register your kids for our Faith Formation/Youth Ministry program! Just click the link below, follow the instructions at the top of the form, and then when prompted, click the link to submit the payment through our online giving page. *Please make sure to submit the Google form before or after submitting payment*. Also please do not worry about registering for My Own Church, as that portion of the registration is not working.

We're thrilled to announce our streamlined registration process through MyOwnChurch for this year's Faith Formation program, catering to children and teens from grades K-12! How do you register? Please follow these steps below:

1. Click on the Registration Link Above and Create Your My Own Church Account: My Own Church is your personalized portal to manage your family’s church records securely and conveniently. Update your information anytime, anywhere, using any Web-enabled device. Visit MyOwnChurch, click "New User," and create your account.

2. Await Authorization: Once you've registered, our dedicated church staff will swiftly authorize your account for seamless access. (May take up to 1 business day for authorization)

3. Register for Classes and Sacraments: After authorization you will get an email that will get you direclty back to the MyOwnChurch website. you can enroll your children and pay any necessary fees for classes and sacraments with ease by following these steps:

           1. Log in to your MyOwnChurch account.
           2. Navigate to the Religious Education tab.
           3. Select the 2024-2025 term.
           4. Add students and provide necessary information.
           5. Complete payment securely online.

Fees for 2024-2025:
- K to 9th Grade: $75.00 for one student, $100.00 for two students, $125.00 for three or more.
- High School Youth Group: No fee.
- Material Fee for Volunteers: Only $15.00 per student for K-9 students; no fees for older students.
*Catechists and Volunteers- If you're volunteering, enjoy reduced tuition by selecting "Catechists/Volunteer" during registration. No online payment is required for  volunteers; simply send in a check for just the student material fee.

- Out of Parish Fee K-9: $200 (If you are not a parishioner here at St. Mary of Nazareth)

Want To Stay Up To Date?

Sign up with our new emailing/texting system, Flocknote. When you sign up with Flocknote, you will be able to stay up to date on all things, Faith Formation or Youth Ministry. The great thing is you can specify what groups you want to hear from! It is as easy as texting: STMDSM to 84576. Follow the prompts, pick which groups you want to hear from, and that's it!
